I am well aware of the risk of handling domestic animals, and with such understanding, I hereby waive, release and forever discharge PAWS PATROL LV and its employees/volunteers, agents or trainers from any and all claims (whether present or future) arising out of my participation in the Volunteer Program. * Yes No Zoonotic Diseases including Rabies: I have been provided with written information regarding the potential exposure to common Zoonotic Diseases carried by Dogs including Rabies, Lyme Disease, Human Ehrlichiosis, Murine Typhus, Plague, and Intestinal Parasites. * Yes No Tetanus Advisory: I have been strongly urged to have a current tetanus vaccination to protect myself should I be cut, scratched or otherwise injured in such a way that tetanus infection could threaten my health. * Yes No Release of Liability: I fully understand that as a part of my volunteer work at PAWS PATROL LV I will come in contact with animals either by direct handling or assisting in their care. Further, I understand that working with animals carries a risk of injury, and that it is possible that I may be bitten, scratched, and/or otherwise injured. I also understand that I may be exposed to canine and/or feline illness and disease and that it is also possible that I could indirectly expose my own pets to such illness and disease. In consideration of this opportunity to volunteer with PAWS PATROL LV, I agree to the following terms and conditions: I will abide by the mission, rules, regulations, and policies of PAWS PATROL LV while volunteering. I agree to be supervised by a PAWS PATROL LV Manager or designee and will work as a team member with all volunteers. I will treat all animals, other volunteers, PAWS PATROL LV staff, and the general public with dignity and respect. While handling or fostering animals in PAWS PATROL LV's care, I agree to become a designated caretaker for those animals. I understand that all information obtained while volunteering about PAWS PATROL LV, any foster, any potential adopter, any animal, or rescue procedures is confidential and should not be discussed outside PAWS PATROL LV without specific agreement from the Volunteer Coordinator. THIS AGREEMENT AND THE TRANSACTIONS CONTEMPLATED HEREBY, AND ALL DISPUTES BETWEEN THE PARTIES UNDER OR RELATING TO THIS AGREEMENT OR THE FACTS AND CIRCUMSTANCES LEADING TO ITS EXECUTION, WHETHER IN CONTRACT, TORT, OR OTHERWISE, WILL BE GOVERNED BY AND CONSTRUED AND INTERPRETED IN ACCORDANCE WITH THE SUBSTANTIVE LAWS OF THE STATE OF NEVADA, WITHOUT GIVING EFFECT TO ANY CONFLICTS OF LAW RULE OR PRINCIPLE THAT MIGHT RESULT IN THE APPLICATION OF THE LAWS OF ANOTHER JURISDICTION.
